Each PCM group is led by a Group Leader. His roles and responsibilities can be categorised according to the 5 purposes, FAITH. The additional category is Leadership and Administration.

Each Group Leader is assisted by an Apprentice Leader and his FAITH Champions.

The Apprentice Leader’s primary responsibility is to assist the Group Leader in shepherding the group with the goal of facilitating either the Group Leader or the Apprentice to step forward to lead another PCM group.

A FAITH Champion is one who assists the Group Leader to advocate one of the purposes of a PCM group. He works under the leadership of the Group Leader.

To provide encouragement and mentoring, a number of Group Leaders are assigned to a Team Leader.

The roles and responsibilities of a Team Leader are much like that of a Group Leader, but now he is primarily focused on Leaders instead of group members. Much of the Team Leader’s ministry is carried out in huddle group meetings and one-on-one meetings. It is expected of the Team Leader to visit each PCM group in the team at least twice a year to be better informed as to how to minister to the Group Leaders.

A huddle is a group of leaders coming together for pastoral care, support and development. The huddle is ideal for modeling group activities, exchanging ideas and envisioning as each leader brings his gifts and experiences to the meeting.
Each huddle may emphasize different components. For example, one meeting might focus on relationship building, another on prayer and yet another on skills development. The needs of the leaders will play a large part in determining what to emphasize in a huddle.

The purpose of one-on-one meeting is to establish a personal, pastoral relationship with each leader. The personal touch assures leaders that they are valued, affirmed and empowered. It also provides the platform for mentoring and discipleship.
The key components to have successful one-on-one meetings are:
- Meeting regularly
- The mentoree’s spiritual journey sets the agenda for the meeting.

All the PCM groups with their FAITH Champions, Apprentice Leaders, Group Leaders and Team Leaders are shepherded by the Parish Leader.

The Parish Leader assists the Pastor in building a vibrant vehicle for Pastoral Care Ministry where no one who is hurting will go unnoticed and uncared for. Much of the Parish Leader’s ministry is carried out in huddle group meetings and one-on-one meetings with Team Leaders and PCM Leaders.
Parish Leader will visit at least two PCM groups each week to be better informed as to how to minister to the PCM Leaders and members.
Team and Parish meetings will be organized regularly to encourage identity, the sharing of lives and the envisioning of groups.

The PCM Vision is communicated and promoted through various means, e.g. Leaders’ meetings and retreats, newsletters like FaithLink for the general church membership; baptism and membership classes, pulpit emphasis, seminars and PCM events etc. These means also serve to cultivate our ownership of the PCM vision and ministry.

The key to the PCM ministry is growing leaders.
PCM leaders need not feel that they are struggling alone in the ministry as support and assistance are available from all levels of PCM leadership including the lay ministry staff and pastors.
Necessary ministry resources e.g. bible study materials, pastoral care and counselling etc. are available from the Parish Leaders.
Continuing Leaders Development (CLD) courses are also organised periodically to provide growth opportunities to the PCM Leaders and members.

There is also the need to grow more leaders so that more PCM groups can be formed.
The primary means to grow leaders is the development of the Apprentice Leader to become a Group Leader. He may then lead his existing group or go forth to lead another group. If the former, the Group Leader is the one who goes forth to lead another group.
This leadership development culture permeates the entire PCM ministry. Pastors are equipping the Parish Leaders to shepherd the people, Parish Leaders are shepherding the Team and Group Leaders to grow into the next level of leadership, Team Leaders are shepherding Group Leaders to grow to be Team Leaders and Group Leaders are mentoring Apprentice Leaders to grow to be Group Leaders. The Group Leaders also look out for the FAITH Champions to become Apprentice Leaders.
